Saint Therese is excited to announce an addition to its senior care organization. In spring 2016 Transitional Care by Saint Therese will open on the fourth floor of the North Memorial Medical Center in Robbinsdale, MN.
Approximately
32 private skilled nursing rooms will provide care to patients transitioning
from hospital to home due to illness, injury or surgery.
“As
the landscape of senior care changes, Saint Therese continues to look for ways
to expand our mission and provide care options that meet the needs of older
adults,” said Saint Therese President and CEO Barb Rode. “Transitional Care by
Saint Therese offers individuals the opportunity to receive quality,
transitional care outside of a care center setting.”
Transitional
Care by Saint Therese provides care that involves an entire team of care
professionals from a variety of departments, working together. Patients may
receive regular visits from specialized physicians and nurse practitioners, in-house
physical, occupational and speech therapists, registered dieticians and the
pastoral care staff.
The team works together with each individual and their family to fulfill
personal needs and goals. Patients will receive a full continuity of care being
connected to the North Memorial Medical Center and will receive priority to
other Saint Therese communities.
